Combining dental and clinical care with wellness and community engagement, HFC offers many ways to get involved!
Primary Medical Care
HFC clinics are centrally located in downtown Faribault and Northfield. Appointments are available Monday-Thursday at our Faribault location and Tuesday-Thursday evenings in Northfield. Our clinics offer an array of primary, preventative, acute, dental and chronic disease care. Please call our clinics at 507-323-8100 to schedule an appointment.
HealthFinders offers dental services on a sliding scale fee including exams, treatment, preventative hygiene, extractions and oral health education at our Faribault clinic location only. We also offer services at the WIC clinic at Rice County Public Health two days weekly.
Our wellness programs offer participants nutrition, exercise and stress reduction classes through our Pura Vida program. HealthFinders diabetes program gives patients extra support in living with their chronic disease. In addition to clinical care, participation in the program includes monthly support groups, education and diabetic supplies. Support groups and classes take place across our community. Patient Advocacy and MNsure
In addition to connecting patients to healthcare, HFC’s patient advocates provide in-depth assistance, connecting patients to resources across the community including: housing, food support, and other resources. Our certified MNsure navigators also provide assistance obtaining insurance. If we don’t know the answer, we can connect you to a broad array of partners who can help.
Community Engaged Wellness
We believe in the inherent capacity of the local community to collaborate together for better health. Placing primary care on an equal plane with patient engagement and wellness, HFC has developed a unique model of healthcare that coordinates care in both community and clinical settings, and unites the clinical team with community-based healers.
Unique Model of Care
Our innovative model of wellness care was recently recognized by the Medtronic Foundation as a way to extend our clinical care team’s reach into the community. This model of care coordination incorporates new and emerging health professions including community health workers, community paramedics, patient advocates, neighborhood organizers, specifically working to reduce the disparate burden of diabetes and heart disease in the underserved community
