Community Wellness
At HealthFinders, we believe health happens in the community, not necessarily in the clinic. The strength of a healthy community is a fundamental belief to our mission of supporting people's inherent capacity to be well.

Wellness Programs
Our wellness programs offer participants nutrition and exercise classes through the Pura Vida Program. HealthFinders’ diabetes program gives patients extra support in living with their chronic disease. In addition to clinical care, participation in the program includes monthly support groups, education, and diabetic supplies. Support groups and classes take place across the community.

Community Health Workers
Community Health Workers (CHWs) are frontline agents of change, helping to reduce health disparities in underserved communities. At HealthFinders, CHWs offer access to services, provide health education, support care delivery, and promote advocacy.

Maternal Health Program
Mayo Clinic Health System and HealthFinders Collaborative have teamed up to provide Community Health Workers (CHWs) as a resource for expecting parents. CHWs work to bridge the patient to their health care system by providing patient advocacy, support, and ensure cultural beliefs are addressed during health care visits.
Pura Vida
The Pura Vida Program offers free fitness and nutrition classes to provide participants with the tools, knowledge, and inspiration to journey toward a healthy lifestyle. Classes are offered in Faribault and Northfield during weekday evenings for anybody interested in becoming more active or maintaining a healthier and happier lifestyle.

Patient Advocacy
In addition to connecting patients to health care, HealthFinders’ patient advocates provide in-depth assistance, connecting patients to resources across the community, including housing, food support, and other resources.

MESA Teen and Family Health
Mejorando la Salud de los Adolescentes (MESA) is a collaborative project, made possible by a grant from the Eliminating Health Disparities Initiative (EHDI). MESA was created to engage the community in reducing the gap between non-Latinx and Latinx teen pregnancy rates and HIV/STIs in Rice County.

Home Blood Pressure Monitoring
The Home Blood Pressure Monitoring Program encourages health care providers in the community to refer patients with hypertension to HealthFinders. If patients are willing, they then connect with our community health workers and we loan blood pressure monitoring cuff to begin our six week evaluation program.

Diabetes Program
The Diabetes Program provides group-based support and education to equip those diagnosed with diabetes with the tools they need to successfully understand and manage their diabetes at home. Program participants meet quarterly to provide continuing social and educational support for diabetes management.

MNsure Assistance
Our certified MNsure navigators provide assistance obtaining health insurance.

Intercultural Training
HealthFinders is dedicated to addressing racial and social injustices by offering opportunities that empower change in our communities.
Local individuals and organizations are invited to take part in HealthFinders’ informative and collaborative intercultural seminars to help improve connections in the community.
