A Special Shoutout to Our Community Health Workers


We at Healthfinders are committed to grassroots community engagement for the health and wellness of the community we serve.  HealthFinders is able to have community health workers (Raquel Rendon and Yakub Aden) because of our HealthRise grant from the Medtronic Foundation.  They work on a daily basis creating, engaging and building ongoing and permanent relationships within our Rice County community and with patients and families knowing that health happens directly where people live.

Because HealthFinders plays a unique role as a community embedded health center, our staff are uniquely poised in both the local Somali and Latino communities not only to identify health issues in their neighborhoods and lead implementation of programs and services but also to provide leadership and facilitate neighborhood-driven health initiatives.
